Appearance of Eyes:
  • Reddened eyes or lids
  • Eyes tear often
  • Blinks, squints, rubs eyes

Complaints When Using Eyes:
  • Eyes hurt/tired
  • Eyestrain
  • Sensitive to light
  • Visual fatigue
  • Blur
  • Double vision

Reading Status:
  • Fatigues easily
  • Loses place often
  • Needs finger to keep place
  • Omits words, endings
  • Re-reads a lot
  • Reduced reading speed
  • Skips lines
  • Decrease in sight words
  • Confuses similar words
  • Reduced comprehension

Writing Skills:
  • Avoids desk work
  • Slow/laborious
  • Dislikes writing
  • Writes uphill or downhill
  • Omits letters, numbers or phrases
  • Misaligns math problems
  • Reduced copying from board
  • Writing poorly spaced/messy
  • Unable to stay on ruled line
  • Size of print varies a lot

Other Problems:

  • Attention issues
  • Motion sickness
  • Reduced eye-hand coordination
  • Poor balance/clumsy
  • Avoids ball sports
  • Inconsistent in sports
  • Discomfort with 3D movies
